Insurance coverage can look a little different for everyone, so it’s important to check directly with your health insurance provider to confirm whether nutrition counseling is included in your plan.

Please take a few minutes to review your coverage and verify your benefits before your first appointment. The easiest way to start is by calling the number on the back of your insurance card and asking:

1. Is nutrition counseling covered under my plan?

To make it easier, provide the following information:

- CPT Codes (Billing Codes): 97802 (initial visit) and 97803 (follow-up visits)

- Preventative Care Diagnosis Code: Z71.3

- Provider Type: Registered Dietitian

- Provider Name: Nicole McMullen Nutrition LLC, Group NPI 1396592051

2. How many visits are covered per year?

3. Do I have a copay, coinsurance, or does my deductible apply?

4. Is a doctor’s referral required?

5. Before ending the call document the date, ask for the representative's name, and ask for a reference number for the call.
